Appendix B. Risk Template

On the following page is a proposed risk template. We are not asking you to fill in forms and manage risks by moving paper, but we want you to consider the set of information you will need to identify, assess, monitor, and reassess risks. We also want you to consider what you need to collect to build a useful risk repository, a database of past risks and their actual outcomes for use in future risk identification and assessment. Nobody’s data is better than your own.
