I'd like to thank my wife Amy for supporting me during the development of this book, (particularly the late nights and weekends that were spent completing it), and indeed throughout our decade plus marriage. I'd also like to thank my parents, Derek and Ann, for installing a sense of independence and self-belief which has taken me many places in my lifetime.
